Did a Terrorist Nearly Kill 300 Because of a Typo?

Some troubling language buried in the declassified summary:

“A misspelling of Mr. Abdulmutallab’s name initially resulted in the State Department believing that he did not have a valid U.S. visa.”

Later, the report says that “delayed dissemination of a finished intelligence report and what appears to be incomplete/faulty database search of Mr. Mutallab’s name and identifying information” were among the human failures that prevented intelligence analysts from connecting the dots.
